Rules differ by state; check yours.

Australian clinics taking consent for a cosmetic treatment.

Skin history and a patch-test question, then the steps for a cosmetic procedure. Two consultations and signed consent come first. Do not take a booking or a deposit until at least 7 days after both. For under-18s, wait at least 3 months between consent and surgery, and arrange an independent evaluation by a psychologist, psychiatrist or GP. A referral is required, preferably from the patient's usual GP, and the referrer must be independent. Screen for body dysmorphic disorder with a validated tool and record that it was done. The questions are not on this form.
