How a paper form becomes a phone form
You do not have to learn a form builder. Send the blanks. We do the rebuild.
1. You send the blank forms
Upload them on this site with no account, email them, post them, or ask for a collection around Perth. Photos of a clean blank are fine. A completed patient or customer form is not.
2. We rebuild and you approve
We set the form out for a phone, using your name and colours. You get a proof. You tell us what to change. That approval is what we treat as the go-ahead to switch it on.
3. You get a go-live pack
A link, a QR code, a website embed snippet, and a short note for the front desk. A printed counter kit is optional. People fill the form on their phone, and a PDF copy can be emailed to you.
Ways to send the blanks
- Upload on the upload page. PDF, DOCX, JPG, PNG or HEIC. Then pick a plan.
- Email them to forms@nopen.com.au.
- Post them. Email us and we will send the Perth address.
- Ask for a collection in Perth. Tick that option on the upload form and we will email you to arrange it.
