Digital forms for medical practices
GPs and specialist rooms collect a lot on paper before a visit. Send us the blank forms you already use. We rebuild them so a patient can complete them on their phone, for example from a QR code at the counter, so the desk can spend less time on clipboards.
Forms we rebuild
Open an example to try the form. The grey text in each box is made up. Nothing you type is sent or stored.
New patient registration
Name, contact and the questions your front desk asks before the first visit.
Open the exampleMedical history questionnaire
The checklist you already use. A yes answer tells the practice to read it. The form does not decide treatment.
Open the exampleConsent to treatment
The practice explains the treatment, its main risks and other options before the patient agrees. Consent can be withdrawn. It includes a phone or video appointment the practice runs, and the person can ask to be seen in person. It does not cover recording.
Open the exampleRepeat prescription request
For existing patients. The medicine, the pharmacy and a note. The practice may need a consultation before it prescribes. This form does not prescribe anything.
Open the exampleSpecialist referral intake
What the referral is for, and the questions you ask before the first appointment. It does not diagnose or recommend treatment.

Do we have to change practice software?
You get a link, a QR code and a snippet for your website. You get an alert when a form arrives, with no answers in the email. We set up how you receive the completed form when your form goes live. If you want completed forms to go straight into your software, ask us for a quote. That works alongside the software you already use.
Can a patient still use paper?
They can fill the form in on a tablet at the desk, or a staff member can enter it for them. You can also keep the paper form at the desk.
