Annual prepay is two months free, and the setup fee is waived. A month-to-month start, with no 12-month term, is $30 more each month and the setup fee is still payable.

Up to 3 forms. No card. No setup fee. We aim to build them within 2 business days of a usable blank copy. If you sign a 12-month term within 7 days of the pilot ending, the setup fee is waived. Pilot details

Anything beyond the forms included in the plan is priced by how complex the paper original is. Simple forms start at $79.

A one-page new-patient form with a single signature is usually Simple. A long form with a body chart or scoring costs more. We tell you the class before we build an extra.
