How blank forms are handled when you send them from this website.

Blank forms you upload

• Files go to a private storage bucket. The bucket is not public, and the site does not hand out a public link.
• Each file is held in quarantine. This website has no download button for those files.
• The upload page requires you to confirm the files are blank, with no patient or customer answers in them.
• We check the file type on the server. PDF, DOCX, JPG, PNG and HEIC only, with size limits.

Finished forms

Answers that people type into a finished form are not stored on this website.

If you sent the wrong file
