Send 1 form of yours, or start from a template. We set it up for phones, with a QR code and your branding, and you use it for 30 days. The setup fee is waived. No card and no automatic charge. The 30 days start on the day we switch your form on for your customers and tell you it is live; sending us your blank form doesn't start them.

- Simple and standard forms fit the free trial. If a form is more complex, we will say so before we build it.
- After the 30 days you choose a plan, or you stop. Extra forms are charged as normal.

We're opening to our first clinics. Upload your blank forms and we'll send you a quote. We'll only switch on a form that collects patient answers once our Australian hosting is live, and we'll confirm that in writing first. Your 30-day free trial starts when we switch your form on and tell you it is live. Uploading your blank forms does not start the trial.
